We could just get Gr4ss to code a plugin that lets an admin know in console with a small notification that a player has died and they automatically /roll upon being respawned?
Sure it could be annoying at start of missions with respawning people, but at least you wouldn't have that "Missing people dying." thing happen.

Dear Jack Jr and Ryan O'Neil,
It is with Immense sorrow that I write to inform you that, your father Jack A. O'Neil Senior, was pronounced deceased during his service aboard the Ulysses S. Grant, under the 112th Morita Battalion, Alpha Company, First Platoon. As your fathers commanding officer, I can say without hesitation that he served not only the 6th fleet, but those appointed below and above him with great honour, courage, and duty. On several occasions, your father stepped above the call, raising his head above the horrors of war, in order to ensure the safety and well being of his soldiers. Whilst I cannot divulge specifics of your fathers passing, I can say without a shadow of a doubt, that he was not only a dear friend to many in the his platoon, but a hero, above all else.
Your father will live in on in the hearts of many, and will forever act as a role model to this battalion.
Please accept my most sincere condolences for the passing of your father.

Dear, Second Lieutenant Kowalski.
Ryan and I are immensely disappointed to hear the news of our Father's death, My father was a man who took the Federation and it's ideals to his core. It was his strong tradition that encouraged us, when we were going through school. We only knew him for two years of our life, when he took a break from Fleet to help us decide what career we would like to go for. His ex-wife... Our mother, loved him very much and even though they were divorced, we both knew they loved each other still. I, and my brother are exceptionally proud however to hear that he upheld a strong reputation, and was a role model for your troops Ma'am. We've heard a lot of things about the 112th, and we saw from the reports that you've taken the first step to beating the Progenitor menace. We wish you luck, and we'll be doing our part and also trying to make our late father proud, wherever he may be.
PS. As the sole heirs to his property and fortune. We wish to donate the immense amounts of wealth he has earned over the years to the War Effort, by supporting the citizens who have earned their retirement from the Mobile Infantry, Fleet, and otherwise. His personal possessions may be handled by a Sgt. Sebastian Bently and Lance Corporal Jennifer Hughes as well as Specialist Short. We would only want his dog tags, and his funeral to be recorded so that we may see it.

Aye, but that confuses me then on how a person could die 4 times and then on their fifth death be asked to roll.
